CONTRACTOR ONBOARDING
Welcome, and thank you for taking the first step to becoming an approved service provider to 360 Facilities.
As part of our contractual obligations to clients, all personnel delivering services on behalf of 360 Facilities must be formally approved prior to commencing work. This includes the issuance of an authorised ID badge, and where applicable, uniform requirements aligned to site or client standards.
To enable onboarding and approval, please complete the Contractor Details Form and upload the required documentation below. At a minimum, the following is required:
- A clear portrait ID photo (for identification badge)
- A valid form of identification (Driver Licence, Passport, Student ID, etc.)
- A National Police Clearance issued within the last 12 months (where available or required by contract)
Upon review and approval of your submission, you will be issued with a Contractor Agreement outlining the terms and conditions of engagement.
Following execution of the agreement, you will be required to complete the Contractor Onboarding Documentation, which includes:
- Relevant business and contact details
- Banking details for payment processing
- Any required site-specific or compliance declarations
- Superannuation and taxation documentation where applicable
All documentation must be submitted and approved prior to being allocated to any service delivery activities.
Contractor Agreement Acknowledgement – Important Notice
By selecting the acknowledgements above and submitting this form, you confirm that you are applying to provide services as an independent contractor to 360 Facilities, and not as an employee.
You acknowledge and agree that:
- Your engagement is subject to execution of a formal Contractor Agreement, and no services are to be performed until written approval is provided
- You are responsible for your own taxation obligations, including income tax and GST (where applicable)
- You are responsible for maintaining all required insurances, licences, and certifications relevant to the services you provide
- You will comply with all Work Health and Safety (WHS) requirements, site rules, and lawful directions issued by 360 Facilities or its clients
- You will wear an approved ID badge and any required uniform when delivering services on behalf of 360 Facilities
- You consent to the collection, storage, and use of your information for onboarding, compliance verification, and service delivery purposes
Submission of this form does not guarantee engagement. Approval is subject to document verification, compliance checks, and operational requirements.
Portrait Image Requirements (ID Badge Photo)
To ensure consistency across all personnel representing 360 Facilities, your submitted photo must meet the following standards:
✅ Acceptable Photo (✔)
- Clear, high-resolution head and shoulders image
- Face centred and facing forward
- Neutral background (white, grey, or plain)
- Good lighting with no shadows or glare
- Neutral facial expression
- No obstructions to the face
❌ Unacceptable Photo (✖)
- Looking away, tilted, or angled head
- Hats, sunglasses, or face coverings
- Filters, heavy editing, or low-quality images
- Busy or cluttered backgrounds
- Cropped images (cutting off head/shoulders)
- Multiple people in the image
- Poor lighting, shadows, or blur

- Non-compliant images will be rejected and delay onboarding
